All N One Healthcare Services is looking for an RN Case Manager for our Home Health Team, the right candidate must have 2 years experience as an RN with one year of experience in Home Health. Must be familiar with OASIS, Medicare guidelines, Quality Management and Quality Assurance.

The position will require you to open cases, supervise a team of LPN's and HHA, and communicate with physicians.
